To mate the diesel ZF S5-42 to the 4BT using the www.destroked.com adapter plate, use the 1994-1997 Dodge Cummins NV4500 flywheel, pressure plate & clutch disc, Ford ZF S5-42 throwout bearing & clutch fork and the Ford 6.0 Powerstroke starter.
I haven't tried this combination, so I can say for sure that it works.
The Cummins/Ford 4BT adapter has the small block Ford bolt pattern. It is my understanding that the small block version ZF will bolt directly to that adapter with the use of the Cummins Ford flywheel. I believe the correct pressure plate / clutch disc / throwout bearing is early 1990's Ford F-250 with the 351W & ZF 5 speed.
